Mirena Coil/Endometriosis/contraception?

HI

I'm a mum to a 3 and a 1 yr old. I suffer with severe endometriosis and have had 3 laperoscopy and lasers plus an adhesiectomy. The last was Nov 08. I saw the consultant last week and she thinks I should go on the mirena coil as its apparently believed to reduce endometriosis and shrink the areas of tissue that the laser can't reach. Obvioously having hadf the procedure 3 times already we do suspect it will continue to recur.

In 03 we were told we'd never have children. I've gone on to have 2... We haven't ruled out a third but iot won'ty be for at least a year. Can the mirena be fitted and then removed within the 5 years if its a/not working or b/ you want to try to conceive? I had the implanon fitted and I bled literally everyday solidly for months and months, despite going tri-cyclic on tyhe pill as well... So I am worried that this could cause me more grief with heavy loss...

In your experience/knowledge is the mirena truly that effective? Is it likely to help? I askbecause its NEVER been mentioned before and actually, before the second lap, the preffered and suggested treatment was heading towards hysterectomy. I refused and sought a different dr who tried a temporary menopause wit triptorelin injections and HRT before another lap but this theory has NEVER been suggested and so I wonder if it will really help?!?! If that makes sense?

Many Thanks for your help and advice....

Dear Charlie,

Thank you for your question.

The Mirena is increasingly being used in gynaecological practice for the management of endometriosis and in answer to your question, yes - it can be removed before the five years that it is licensed for, if you experience any problems or decide to try to conceive.

Nuisance bleeding with the Mirena tends to settle within four to six months for most women who experience it, so it is important not to be hasty about removal. Although both the Implant and the Mirena have progestogen-type hormones, you will not necessarily experience the same problems that you experienced with the implant. So please do try it if it has been recommended.

In women with endometriosis, the Mirena is thought to work by suppressing the growth of cell deposits of the lining of the womb that are found outside the womb, for example in the pelvis. It may also reduce any associated inflammation that gives rise to formation of adhesions as well as pain.

Management of endometriosis is not always a straight-forward one and may require more than one approach. I suggest that you have the Mirena inserted as recommended by your gynaecologist who will certainly follow this up and consider other options if this does not work for you. With the Mirena, you will also have the added benefit of safe and effective contraception.

I hope this helps.

Best wishes, Dr Valerie

Hi Charlie

Hope you don't mind me adding my bit!

I had a terrible time with the implant but found the Mirena much better (will be my first choice of contraception after childbirth). Eased my heavy, painful periods and didn't affect my mood the same way Implanon did. Have also been talking about them at work today and everyone has said that although insertion can be uncomfortable, and a day or two after you may feel so sore and wonder why on earth you had the thing inserted, we all agreed that it is soooo worth it and a wonderful contraceptive and relief for problematic periods!
